By signing this form, I agree to the following: I understand that tuition is due no later than the first day of each month and that other expenses are due as stated in the NDA “Important Information handoutâ€. I understand that I will be charged a $15 late fee if my payment is late and my dancer may be asked to sit out of class. If I have chosen to pay with credit card, I understand that I must provide updated account information if my card expires or if my account has been compromised. If charges to my card are declined for any reason, I will be charged a $10.00 service fee. I understand that if I choose to pay by check and any of my checks are returned for non-sufficient funds, I will be charged a $30.00 NSF fee. If any of my checks need to be re-deposited for any reason I will be charged a $10.00 fee. Northfield Dance Academy, including its director, teachers and other staff are not responsible for any injuries or loss of property while the student is attending class, traveling to and from the studio, or attending/performing at studio functions. I give Northfield Dance Academy permission to use my child’s photograph and/or name for promotional purposes only (i.e. brochures, web site, newspaper ads, t-shirts, etc.).
