I, the parent/guardian of the student (the "Child"), hereby consent to the participation of the Child, as a student in the Anchorage Classical Ballet Academy's (ACBA) Ballet/Dance programs. I/We understand that physical contact and placing of the body by the dance teacher is necessary for proper instruction.
Parental Waiver and Release (Alaska Stat. 09.65.292)
I/We understand that dance instruction involves physical movement and coordination, stretching of muscles and physical exertion, all of which presents a risk of physical injuries associated with such physical movements, Including Injuries from falls, muscle strains, overuse injuries involving an ankle, leg, foot or lower back, hip injuries: snapping hip syndrome, hip impingement, labral tears, hip flexor tendonitis, hip bursitis and sacroiliac joint dysfunction, foot and ankle injury, Achilles tendonitis, trigger toe and ankle impingement, knee injuries: patellofemoral pain syndrome, stress fractures, and the possibility of developing arthritis in the knee, hip, ankle and foot.
I/We acknowledge and agree that I/We further discharge and release ACBA, Its Board of Directors, officers, and employees from liability for injury, loss, damage, obligation, expense, or penalty sustained by the Child arising out of or in connection with the Child's participation as a student in ACBA Ballet/Dance programs; provided, however that this Release and Waiver of claims does not release ACBA from its own intentional or reckless misconduct.
If an issue should arise I/we will talk to ACBA staff in a calm and respectful manner so the issue may be resolved as quickly and professionally as possible. I/We will endeavor to be a parent with a problem not a problem parent. ACBA reserves the right to expel any Child whose behavior or parent's behavior is deemed unacceptable. In the event that a Child is expelled, there will be no adjustment or refund of tuition. Drug or alcohol abuse on or off ACBA property, will lead to immediate dismissal.
Registration/Enrollment
Once a dancer is registered for a season class they are committed for the entire season. Registration fees are due at the time of registration. If no tuition payment has been made within the 14 days, the Child will be withdrawn from classes. ACBA reserves the right to close classes or change class times. ACBA reserves the right to determine the placement level for every student regardless of prior training.​ Your child’s tuition payment holds their spot in class. There are no tuition adjustments for missed classes. ACBA does not offer makeup classes.
Should you need to adjust your or your child's class schedule, an Add/Drop Form must be completed with ACBA’s front desk.
If a dancer wishes to withdrawal prior to the end of the Academy Season you must apply in writing by the 15th of the month for the withdrawal request to be considered. The remainder of the Season tuition will be forfeited. Refunds and/or credits will not be given. Partial refunds will not be granted, except in cases of prolonged injury or illness with a doctor's note.
Tuition/Fees
Registration fees are due at the time of registration. Tuition is calculated based on the number of classes per year, then divided into equal payments. Full installment payments are due regardless of holidays, breaks, or snow days for the season. There is a non-refundable production fee for all students performing in The Nutcracker, Spring Production and/or the Spring Recital. There is one costume fee per performing class. All production/recital fees and costume fees are non-refundable.
All tuition/fees may be paid in one of four ways for the season:
Payment in full at the time of registration
Payment in three installments - due August 1, November 1, and February 1
Payment in ten installments - due August through May on the 1st
Payment in eleven installments - due July through May on the 1st
Tuition/fees must be paid through autopay with a credit or debit card. All NSF/declined transactions will be charged with a $25 late fee. Registration fees, tuition, recital fees, production fees, and costume are non-refundable once processed. There will be no partial month or yearly refunds.
If no tuition/registration fee payment has been made within 5 days of enrollment, your child(ren) and/or self will be withdrawn from classes.
Medical Release
I understand that it is my responsibility to consult with a physician prior to/regarding my Child’s participation in any ACBA program. My Child has no medical condition that would prevent participation. I have informed ACBA of special medical considerations, including allergies, and past injuries.
I consent to emergency medical attention deemed appropriate for the Child(ren) or myself while participating in ACBA programming. I/We hereby waive, to the fullest extent allowed by law, on behalf of myself, my spouse (if applicable), and my child(ren), and liability to ACBA and any of its agents or employees, arising out of such treatment.
Media/Photo Release
I/We hereby irrevocably consent to and authorize the use and reproduction by ACBA of any and all photographs, recording, videos, and/or reproductions of likeness of the Child(ren) person or characteristics (reproductions) or myself which have been secured by or for ACBA, for any promotional purposes whatsoever, without compensation to the Child(ren) or his/her representatives or myself. I/We hereby release and discharge the ACBA from any and all claims and demands arising out of or in connection with the use of media, including any and all claims for libel.
I have carefully read, understand, and agree to all of the aforementioned waivers and policies.
