I and/or my child have chosen to participate in dance/ fitness instruction given by Haley Clark Dance Company. I acknowledge that I understand the nature of the activities I will be participating in and that I/my child am/is in the proper physical condition and capable of participating in the related activities, understanding that Haley Clark Dance Company is not in any way responsible for making such a determination.

In consideration of my enrollment in any dance/fitness instruction, I understand and agree on behalf of myself and/or my child, to release, hold harmless, and discharge Haley Clark Dance Company from all claims, costs, liabilities, expenses or judgments, including attorneys’ fees and court costs for any occurrences in connection with any instruction. I assume all risks in connection with any instruction and further release Haley Clark Dance Company and its owners and employees from liability for any injury sustained by myself/child while I/my child am enrolled in any dance/fitness instruction, including all risks reasonably connected with such activity whether foreseen or unforeseen.

I understand that Haley Clark Dance Company is not responsible for personal property that is lost, damaged or stolen while I/my child am on Haley Clark Dance Company property. I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself/my child and that Haley Clark Dance Company does not provide accident or health insurance for those participating in its instruction.

I authorize and agree that Haley Clark Dance Company may take and use photographs, videos or likenesses of myself and/ or my child as needed for its record-keeping, advertising and/or public relations projects and that I have no rights to the same and will not be compensated for the same. My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation. I have received the registration information and will abide to the policies.

I understand the risks of COVID-19 and I knowingly and willingly consent to attend/ my child to attend classes, events, facility, etc. I understand that due to the characteristics of the virus and Haley Clark Dance Company staff that I/ my child have elevated risk of contracting the virus simply by being in the Haley Clark Dance Company facility. If I/my child contracts COVID-19, I will inform the Haley Clark Dance Company as soon as possible and will not hold them responsible for me/ my child contracting COVID-19.

I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND AGREE TO ALL TERMS.
