Participation Release and Waiver of Liability: I have chosen to participate in fitness or have my child(ren) participate in the instruction given by Mayeisha's Fine Arts Studio LLC ("MFA Studios"). I acknowledge that I understand the nature of the activities either I or my child(ren) will be participating in and the possibility that despite precautions, accidents, and/or physical injury may occur. I agree to release and hold harmless MFA Studios including its' teachers and staff members from any cause of action, claims, or demands now or in the future. I will not hold MFA Studios liable for any personal injury or any personal property damage or loss, which may occur on the premises before, during, or after classes or which may occur at any MFA Studios sponsored event outside the studio. Furthermore, I agree to read and follow the class and facility policies and take full responsibility for my and/or my child(ren)s behavior in addition to any damage I or they may cause to the facility utilized by MFA Studios.
Photo & Video Release: I authorize MFA Studios, its representatives, and employees the right to take photographs, videos, or likenesses of me and/or my child(ren), and/or property in connection with MFA Studios. I authorize MFA Studios to copyright, use, and publish the same in print and/or electronically for the use of record-keeping, advertising (to include website development), social media, and/or public relation projects. I agree that MFA Studios may use such photographs and/or videos of me and/or my child(ren), with or without my name and for any lawful purpose. I acknowledge that I have no rights to the same and will not be compensated for any use.
Annual Membership Fee: An initial registration fee of $150 will be applied to your account at the start of your/your child's membership. Thereafter, a fee of $99 is charged once yearly and is non-refundable. This fee covers the administrative costs involved in maintaining your account (including staff coverage and online access), course layout by demand, instructor contracting, classroom allocation, and class scheduling for the preceding year. Registration fees for students enrolling after the start of the season (August 1) are due upon enrollment in order to secure a spot in class.
Perpetual Enrollment: Membership with MFA Studios is offered on a perpetual basis. This means that the sessions do not end, they continue from month to month, perpetually. You may end your lessons at any time by following our Withdrawal policy. If you do not withdraw, your enrollment will automatically be renewed in August for the next year. Your monthly payments will be recalculated, and the Annual Fee will be added. If a student is promoted to the next level, we will notify you of the schedule change at that time, no action is needed on your part to continue your Perpetual Enrollment.
Dress Code: It is important that students are properly dressed for their attended classes. As you can understand, young dancers are sometimes distracted by their own dancewear. Large skirts and/or tutus sometimes become the dancer's focus instead of the instructor. Learning to dance and work together as a team can be more easily achieved by removing the distraction of different dancewear. It is also important for Instructors to be able to see the dancer's bodies so that proper positioning and technique can be learned to assure a young dancer can move forward in their development with a strong foundation. As such, a simple leotard, tights, and skirt combination are the best alternative. Complete details of our Dress Code are broken down by age group and/or class, as appropriate, and may be found on our website: www.mfastudios.com, under the Dress Code Tab. The dress code has been set up to encourage color separation for growth to the next level of dance-giving young dancers a goal they can strive to achieve. As well as assure adult students are dressed appropriately to prevent distraction or potential injury.
Communication: Our primary methods of communication are email, our website, and Facebook. Please be sure we have your current email address and visit our website and Facebook page regularly. We do not hand out paper reminders and our staff is not required to make announcements in class. If you have any questions or concerns, ask the front desk staff or email questions to frontdesk@mfastudios.com.
Refund & Withdrawal Policy for Dance, Acro, Cheer, non-competition Classes/Camps:
Non-attendance does not constitute withdrawal from our program. Notification to withdraw from a class must be received in writing to MFA by the 15th of the month PRIOR in order to stop tuition payment for the following month. No exceptions. The student's intention to withdraw may be submitted via email or by a letter submitted in person to front desk staff. Upon receipt of your written request, you will receive an MFA Studios Withdrawal Request Form. MFA Studios Withdrawal Request Forms must be completed and submitted, via email or in-person to the front desk staff, in order for the withdrawal to be complete. The student may continue to attend their scheduled class(es) during the 30-day period, payment of tuition is still expected and may be made by the continuation of electronic payments or by direct payment through front desk staff. The total value of lost dance days due to holidays or holiday breaks has been considered and is reflected in the monthly tuition amount. There are no additional refunds or credits for missed dance days due to holidays.
Make-Up Policies: If a class is missed due to illness, vacation, or we are closed due to a holiday, inclement weather, or instructor availability we encourage you to participate in a Make-Up Class within four (4) weeks of the absence. This is a great opportunity to try a new class that you've been interested in or take a class that is similar to or the same as your current class. Make-up lessons must be pre-arranged as some classes may have size restrictions. Please contact the front desk to review the schedule and register. Make-ups cannot be used in the substitution of tuition payment.
Referrals: We love referrals! For every person or family, you refer to MFA Studios you will receive a $55 credit as our thanks! Please have the new student refer to you by name at registration and upon registration of the new student, we will apply a $55 credit to your account!
Payment Methods: Our required method of payment is by pre-approved, Auto-Pay, directly debited from your checking account or charged to your credit card on the 25th of each month prior. A convenience fee of 3% will be added to the total amount. By choosing this option, you authorize regularly scheduled payments to be made by MFA Studios. This permission will remain in effect until you notify us in writing to terminate this authorization. If for any reason, your payment does not go through, we will notify you and attempt to process the payment again, however, a $25 late fee will be added to your account. By entering your payment information via the MFA Studios payment website, you authorize MFA Studios to initiate electronic payments for the balance due on your account. You understand that payments will be processed on the 25th of each month as long as there is a balance due. You understand that the payment amounts may vary as classes are added/dropped or as other charges/payments are applied to your account. If you want to pay in person via check or cash, please submit payment by the 24th to avoid using the card on file. If payment is not made BEFORE the first of the month, a $25 late fee will be assessed.

If you would prefer to speak with the Director directly, please leave a message with the staff at the studio front desk or make your request directly to her via email. She will call you within 1 business day of your message or request.
Collection Process: In the event your account becomes delinquent and all attempts to remedy the balance fail by working directly with MFA Studios personnel, MFA Studios reserves the right to place your outstanding balance with an outside collection agency or law firm, any applicable collection fees, attorney fees, court costs¸ and/or court-ordered interest will be your responsibility.
Performance & Costume Fee: The performance fees are additional and include 1 Costume, recital studio, group class photo, and video download. Dancers in the Winter performance will pay their Performance Fee by September 15th. Spring Recital Performance Fees will be due November 15th.
Additional fees for costumes, performances, competitions, and studio wear must be made payable by check to MFA Studios. Payments made by electronic check or credit card will incur a 3% service charge.
Private Lessons: Private lessons are available in all styles and levels of dance. Private Lessons are for people who wish to advance and learn more rapidly and/or have specifics that they would like to address on a one-to-one basis. This intense and personalized attention is by appointment only and is scheduled based on the Instructor's availability. Private lesson fees are due at the time of registration and are non-refundable. Missed classes will be rescheduled, at no additional charge, if 24 hours' notice is given. If less than 24-hours' notice is given the registration fee is non-refundable and rescheduling will not be permitted. The 24-hour notice policy is non-negotiable so that MFA Studios and the Instructor have ample time to rebook the appointment and classroom space.

Fitness Membership Cancellation Fee: In the event, a member withdraws from the MFA Studios Fitness program, a $250.00 cancellation fee will be applied to your account. We contract our instructors based on your registration and determine class schedules by the number of students participating in each class (not based on attendance). Please see the conditions of Withdrawal and Cancellation Fee Waiver exceptions detailed below.
We understand that, in special circumstances, a withdrawal request may be out of a member's control. In the event, the withdrawal request is following employment relocation, injury, or unforeseen health issues MFA Studios will waive the $250 Fitness cancellation fee with a completed MFA Studios Withdrawal Request Form and documentation provided by the employer or healthcare professional.
Fitness "No show/ No call" Policy: You must notify us via email at Dance@mfastudios.com at least TWO HOURS in advance if you are unable to attend a class you signed up for or you will be charged a $5.00 "No show/No call fee". We have a waiting list for our fitness classes and would like to give someone else the opportunity, with advance notice, to attend a class.
Automatic / Recurring Payment Plan
Here's how the Direct Payment Plan works:
You authorize regularly scheduled payments to be made. Your payments will be auto processed on the 25th of each month for the next month's membership. Proof of payment will appear on your account. If for whatever reason, payments cannot be processed to your payment information on file and your account balance remains overdue, a $25 late fee will be accessed on any balance not paid BEFORE the 1st. I authorize Mayeisha's Fine Arts Studio LLC to initiate electronic payments for the balances due on my Mayeisha's Fine Arts Studio account. I understand that payments will be automatically made throughout the year for any balance due on my account. I understand that the payment amounts may vary as classes are added/dropped and as other charges/payments are applied to my account.
