WAIVER OF LIABILITY
I hereby give permission for my student to participate in dance classes and other programs at Ballet Chelsea, whether in-studio, or virtually. I understand that I am enrolling my student in a program of physical activity and confirm that my student is in good physical condition and does not suffer from any physical or mental condition that would prevent or limit participation in Ballet Chelsea programs. If I am the parent of a younger student in adaptive dance, I or a designated care giver will be in the studio with the student at all times, and will be responsible for the safety of the student. I release Ballet Chelsea, its directors, officers, employees, contractors and agents from any and all liability, and waive the right to take legal action against Ballet Chelsea and the foregoing representatives, for any injury sustained in its facilities or at any Ballet Chelsea event.
I know and acknowledge that these are uncertain times. The understanding of the risks of COVID-19 by the scientific community is continually evolving.
In consideration for providing classes and instruction, by signing below I agree to accept all responsibility for the risk that my student may be exposed to others here who have contracted COVID-19 or been exposed to someone else who has it, and the risk that my student may contract COVID-19 herself or himself, while arriving at, leaving from, and during her/his time at Ballet Chelsea. I acknowledge and accept that I am choosing to involve my student in activities at Ballet Chelsea, and Ballet Chelsea, its Board, executive staff, teachers, and all other staff (whether employees or contractors), will not be liable should s/he contract COVID-19 or come into contact with another person who may be infected or have been exposed to it.
