I have read the registration information on the website and understand and accept the policies as outlined. I understand that I am responsible for tuition payments as described. I certify that the student is in good health, and is capable of vigorous activity. I agree that I will not hold the Penny Askew Ballet Academy or any faculty member responsible for any injury or loss of property sustained by myself or my child in connection with the registered classes. I hereby give my permission for Penny Askew Ballet Academy to take photographs for promotional uses for the Academy.
