Liability Release Form and Assumption of Risk

I/we realize that participation in dance classes and activities could potentially result in some possible personal injury. Despite precautions being taken by the studio, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces
used by Aspire Kinetic Arts.

I/we agree to release from responsibility Aspire Kinetic Arts including all teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I/we will not hold Aspire Kinetic Arts liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken
bones, concussions or death or any personal property damage/loss, which may occur on the premises
before, during or after classes.

Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our
behavior in addition to any damage I/we may cause to the facilities utilized by Aspire Kinetic Arts.

My acceptance serves as a release and assumption of risk for myself and any members of my family who
have accompanied me to these activities, including those under the age of 18 for whom I am a parent or
legal guardian.

Aspire Kinetic Arts (Aspire) is committed to the health and well-being of our students and community. In accordance with guidance from state health officials, please follow these procedures while at Aspire:

1. Bring a water bottle for your own use or use our drinking fountain at your own risk.
3.Do not come to class if you feel unwell, have any COVID-19 symptoms, or have come in contact with a confirmed case of COVID-19.
4. If you do test positive for COVID-19 or another contagious illness and you/your student has been to Aspire lately, please contact us with that information.

COVID-19 Waiver:
I have read and understand Aspire's COVID-19 polices for students and families. I will adhere to the COVID-19 policies to the best of my ability. By signing below, I affirm that I consent and commit to Aspire's COVID-19 policies. I understand that is always some risk of contracting COVID-19 or any contagious illness and assume the risk of participating in camps, intensives, or classes at Aspire on behalf of myself, my family, and any personal representatives; I release Aspire, its owners, and its faculty from any liability now or in the future if I contract COVID-19 or any variant thereof, however caused, during or after participating in activities at Aspire.
