Parent/Guardian Health Statement
I certify that I am the parent or legal guardian for my child(ren). My child(ren), family members, and/or myself are healthy and physically able to participate in all Project Performing Arts classes and activities. If needed, I authorize and request the Director of Project Performing Arts to secure necessary emergency care and treatment for my child.
I hereby waive, release and forever discharge Project Performing Arts from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Project Performing Arts activities, whether or not damages or loss is due to negligence
Photo Release
I hereby give consent to the use of my child(ren)'s, family member's, or my own portrait by Project Performing Arts (PPA) for the purpose of advertising on the PPA website, Facebook page, Instagram app, and for any publicity purposes in all media including, but not limited to, television, website, radio, and print publications.
If you do not give consent to the use of your child(ren)'s, family member's, or your own portrait or picture by Project Performing Arts for any advertising or publicity purposes please send an email to ppa@projectperformingarts.org to notify us.
Program Policy Agreement
I understand that Registration is Annual (PPA school year/season) and is non-refundable.
Tuition refunds are made based upon special circumstances. Requests are made by email, and if approved will be refunded within two weeks. Refunds are issued by check.
I understand the importance of the proper dress code, shoes and items needed for specific classes. I agree to make sure my child/children are prepared for all classes.
I understand PPA class curriculums are based upon 34-instructional weeks. Although there are 2-semesters, it is the expectation that student’s register for the 2nd semester to complete the year-long instruction.
I understand that dance classes may include, without limitations, stretching, barre work, jumping, dancing with props, across the floor and center floor combinations and other physical activities.
I further understand that all the activities of acting and drumming classes also involve physical activity and some degree of muscle strains and pulls. In the event of such, an incident report will be filed, and parents will be notified.
I understand if I or my child has a question or would like an explanation regarding training that the PPA Creative Team welcomes questions.
I understand if a specific situation occurs the Creative Team is ready, and will do the best to rectify the situation.
I understand that bullying and cyber bullying are contrary to and against Project Performing Arts mission and core value.
I understand that in some situations instructors might not be able to complete the semester. I understand that a new instructor will be placed in the class to finish the year. The PPA Creative Team will do their best to help students with the transition.
I will make PPA instructors aware of any physical accidents that occur outside of Project Performing Arts Program, and that might impact a student’s participation in class.
I understand that Project Performing Arts is not responsible for the loss or damage of personal property.
I agree to be responsible for reading all PPA correspondence, and respect deadlines.
I hereby acknowledge that I have read the statements above and agree to participate accordingly.
