RELEASE FROM LIABILITY: I do hereby release Ballet Academy of South Texas and its staff from any liability occurring on or around studio premises, or at any function held at other locations in connection with the dance classes in which my student is enrolled. I declare that my student is in good health and can participate in dance education classes. Given the nature of dance classes, and with the knowledge that injuries sometimes might occur, I have taken the necessary steps to obtain accident, health, or hospitalization insurance which would cover any sustained injury. In the event of an injury or emergency when I cannot be contacted, I give my permission for you to obtain medical services for the student named above.

PHOTO RELEASE AGREEMENT: I grant permission to publish classroom and/or performance photos of my child on the studio website, social media or other marketing materials. I give Ballet Academy of South Texas the perpetual, royalty-free right to use the photo(s) for marketing and/or advertising purposes. I understand that my child’s photo will be available to the general public. I further understand that Ballet Academy of South Texas assumes no liability or responsibility whatsoever concerning any consequences of such use. I understand that if I give notice that I object to any particular picture used, it will be removed as soon as possible.

AUTHORIZATION FOR CHARGES: I authorize Ballet Academy of South Texas to enroll my student in dance class(es), and I accept responsibility for the payment of tuition for those classes for which the student is registered. I understand that classes with an enrollment of less than six pupils are subject to cancellation. I understand that registration fees are non-refundable and that there are no refunds or deductions for classes not attended. I understand that if I do not give written notice of the student’s withdrawal from classes by the 10th of the month, I am obligated to pay the next month’s tuition payment.

COVID RELEASE: I understand and acknowledge on behalf of myself and my dancer that COVID-19, the disease caused by the coronavirus, was declared a global pandemic. The symptoms of COVID-19 include fever, chills, cough, sore throat, muscle pain, headache or loss of taste or smell. COVID-19 can result in severe harm including death. More information regarding COVID-19 can be found at https://www.cdc.gov/coronavirus/2019-nCoV/index.html. You are encouraged to read the information prior to signing this release. I further understand and acknowledge on behalf of myself and my dancer that my dancer may come into contact with, be exposed to or contract COVID-19 while at Ballet Academy of South Texas.

On behalf of myself and my dancer, I accept and assume such risks and responsibility for the losses and/or damages related to my dancer’s potential exposure to or contraction of COVID-19, including death, however caused and whether caused in whole or in part by the negligence of the Ballet Academy of South Texas. I, on behalf of myself and my dancer, agree to release and hold harmless Ballet Academy of South Texas and their respective parent, subsidiary and affiliated companies, partners, and employees (collectively, the “Released Parties”), with respect to any liability, claims, demands, causes of action, damages, loss or expense, including court costs and reasonable attorney’s fees of any kind or nature which may arise out of, result from, or relate to COVID-19 during my dancer’s participation at dance, including claims for liability caused in whole or in part by the negligence of the Released Parties.
