By signing this waiver, I understand that Lacey Performing Arts Center can not be held liable for any accident or injury that my child(ren), child I am overseeing or myself have on the premises. This includes in the studio and outside of the studio located at 116 1st Dr. SW New Philadelphia, OH 44663. By signing I have provided all correct and updated information about myself and my child to Lacey PAC. I am aware that I (the parent, guardian or one in charge of the student or students attending Lacey PAC) hold all responsibility for any injury, accident, sickness or other medical or non-medical incident at Lacey PAC. Any damage done to the property, accident or not, is my responsibility, this includes but does not exclude, floor damages, fixtures, rooms and any other valuable property to Lacey PAC. By signing this I understand I cannot sell, or profit off anything Lacey PAC, including dances, music, attire, marketing, the name, the brand and anything else Lacey PAC uses. By signing this I allow my student(s) or child I am overseeing to participate at Lacey PAC with full acknowledge of all that is stated above. Lastly, I give permission to Lacey Pac to use photos or videos of my student(s) on social media, website or for any marketing aspects.
*Lacey Performing Arts Center POLICIES*
Registration- An annually nonrefundable $20 fee per student will be charged at registration. The fee will get your child their first free t-shirt that will be worn in their end of year recital. The fee along with the first month tuition is due upon registration.
Tuition- No tuition will be handed to anyone other than the owner Lacey Herbert-Stephen or a secretary during office hours.
Tuition may be paid online, at the front desk or in drop off box.
You will be responsible for all tuition until the end of the season or until a formal cancellation is filed.
ALL tuition must be paid by the 1st of every month. If tuition is one week late you will receive a warning, if your tuition is not paid by the 10th of that month, you will then be charged with that month’s tuition and an additional late fee of $25 for each week that tuition is late.
There will be a $35 charge for any returned checks. Tuition is not pro-rated or returned for any reasons such as illness, vacation, weather, or prior commitments.
If a class is cancelled there will be a scheduled make up day.
If your account is ever behind it could result in a suspension for that student depending on the length of time it will take to get the account caught up on the tuition. All accounts must be paid in full every month and every year to participate in any events the center does.
There is a TUITION BOX located outside of the office. Please feel free to drop off your tuition there if no one is in the office; please put it in an envelope with your name on it!
Cancellation- Students may withdraw/dropout from their class within 30 days of the registration date. They must give a 1 week notice before the cancellation in writing with the front desk. If canceling after the 30-day grace period they will be charged a cancellation fee of $15 per class. Some exclusions apply.
Responsibilities- Students will follow all dress codes that their classes and teachers require. The student and parent will be responsible for picking up newsletters, paying tuition and checking in on time.
A newsletter will be printed once every month or emailed.
The newsletter will be in the lobby and emailed to every parent unless a written notice is given to send by mail.
Photo Release- I allow Lacey PAC to publish photographs of my son/daughter and of family. Pictures could be published on website, brochures or any other marketing aspect. A written note must be given
to the director or secretary if you decline this photo release.
Selling- There will be NO selling or advertising of any merchandise or services outside of or at Lacey PAC, unless authorized by the director Lacey Herbert-Stephen.
Attitude- By initialing/signing this I promise to promote a good attitude around any students or parents and at any events with Lacey PAC.
If any negativity is shown that could hurt the Lacey PAC in any way I
understand I will be asked to leave with no refund of any payments.

Dress Code is very important here at Lacey PAC; all classes will have a specific dress code on our website/facebook and will be given to the kids at registration. Please make sure that your child(ren) follows this dress code the whole year they are at Lacey PAC.
Ballet has one of the strictest dress codes in the studio; all students MUST follow the Ballet dress code EXACTLY! If a student arrives to class out of dress code the Ballet Instructor may ask them to sit out or leave.
Dress code enforcements teach students responsibility and respect toward their teachers and other classmates!
All important dates such as performances, recitals, snow days, Christmas/Thanksgiving vacation will be posted on our website/facebook and emailed, please check your email often.
Our studio will always follow New Philadelphia Schools on vacation breaks and with snow days!

*No drop out fees within first 30 days, after 30 days is a $15 drop out fee per class, some exclusions apply.

END OF CLASSES: Classes will END in late May, official date TBA
DO NOT PARK IN THE SPACES DIRECTLY ACROSS LOBBY DOORS! This is private property and violators will be towed! Please park along Allen Lane with parking meters (they don't check them after 430p). In Ricardos parking lot, along broadway (downtown parking spaces) or behind the building in the Quaker Cinema parking lot. Please be courteous to the local business if you are parking in their spaces!
