I have received or will obtain a copy of the Danse Elan LLC policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Danse Elan LLC does not issue credit and/or refunds for class(es), workshops or camps missed due to holiday, vacation, illness, weather, etc or credits or refunds for costumes and recital fees. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Danse Elan LLC, and I voluntarily assume the risks associated with such participation.

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless Danse Élan and its staff from and against any and all claims for personal injuries or damages of any kind arising from participation in any of Danse Élan's programs. Further, I authorize Danse Élan's staff and faculty to seek emergency medical help for myself or my child(ren) if it becomes necessary. I realize that every effort will be made by Danse Élan's staff to contact me in the event of a medical emergency involving my child(ren) and I agree to indemnify and hold harmless Danse Élan personnel in seeking care for me or my child(ren).

WITHDRAWAL/REFUND POLICY: In the event that a student must withdraw from dance class/es, workshops, camps, or related activities, written notice must be provided 5 days prior to the next tuition due date in order to discontinue automatic payments. Registration fees, costume fees, recital fees and tuition for classes and camps are non-refundable. There are no exceptions to this policy.

PHOTO/VIDEO CONSENT: By your signature, you agree that Danse Élan may use your child(ren)'s photograph/video in the routine promotion of its classes and activities and for other non-commercial applications.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19: In consideration participating in Danse Élan LLC's dance program, art program, camp program and other related events and activities, I acknowledge, appreciate and agree that participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. I acknowledge that such exposure or infection may result in personal injury, illness, permanent disability, and death for me, my child(ren) and other family members. I understand that becoming exposed to or infected by these diseases may result from the actions, employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury (including but not limited to personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at the studio or participation in studio programming. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the studio, its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the studio, it's employees, agents, and representatives, whether an infection occurs before, during or after participation in any studio program or event.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

PUBLIC GRIEVANCE WAIVER

I acknowledge the policies and guidelines regarding the airing of grievances at Danse Élan and agree to the following:

1. I understand and agree that publicly airing grievances on social media platforms, in group messages, or in the studio lobby is strictly prohibited. This includes, but is not limited to, posts, comments, or messages that criticize the studio, its staff, or its students.

2. I agree to address any concerns through the appropriate internal channels, namely:
a. Email: I will send a detailed description of my concern to INFO@DANSEELAN.COM.
b. Meeting: I will schedule a private meeting with the studio director or appropriate staff member by contacting INFO@DANSEELAN.COM.

3. I acknowledge and agree to the non-retaliation policy and will not engage in retaliation against any individual who raises a concern in good faith.

4. I understand and agree that all grievances will be handled with the utmost confidentiality, and the privacy of all parties involved will be respected throughout the resolution process.

By signing below, I acknowledge my understanding and agreement to adhere to these guidelines and support a supportive and positive environment for everyone at Danse Élan.
