WAIVER AND RELEASE OF LIABILITY

I hereby acknowledge and agree to the following terms and conditions in consideration for being allowed to participate in activities at Premier Cheer Elite, located at Unit 11, 103 Stenhouse Drive, Cameron Park:

Assumption of Risk: I understand and acknowledge that cheerleading activities involve inherent risks, including but not limited to, the risk of injury or death. I voluntarily assume all risks associated with my participation in cheerleading activities at Premier Cheer Elite.

Release of Liability: I hereby release, discharge, and hold harmless Premier Cheer Elite, its owners, employees, coaches, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action that may arise out of or relate to any injury, illness, or damage to my person or property, including but not limited to claims arising from the negligence of Premier Cheer Elite.

Medical Authorization: In the event of an emergency, I authorize Premier Cheer Elite and its representatives to obtain and consent to medical treatment for me, including hospitalization, surgery, or other medical procedures, if deemed necessary.

Compliance with Rules: I agree to comply with all rules, regulations, and instructions provided by Premier Cheer Elite and its staff during my participation in cheerleading activities.

Media Release: I grant Premier Cheer Elite the right to use my name, likeness, voice, and image in any photographs, videos, or other media for promotional and marketing purposes.

I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY, AND I VOLUNTARILY AGREE TO ITS TERMS.

COMP TEAMS: I understand that my fees must be paid through the PCE Portal ON THE FIRST MONDAY of each month.

RECREATION CLASSES: I understand that my fees must be paid through the PCE Portal ON THE FIRST MONDAY of each term before classes begin.

Late fees are not accepted. If you do not pay your fees your child will not be allowed to attend class. If you drop your child off and fees are not paid, they will have to sit out. This is a rule we will be strictly enforcing this year.
