In consideration of receiving the services as described above, I agree to release, hold harmless, and indemnify The Ballet & Dance Center, LLC, its employees, agents, instructors and officers from any and all claims, actions, causes of action, damages, and attorney fees, arising from personal injuries and/or infection due to Covid-19 that may be sustained resulting from participation in the programs provided by The Ballet & Dance Center, LLC.

I, hereby grant permission to The Ballet & Dance Center, to take photographs, videos or other media of the "student(s)" noted on this account for use on its website, social media platforms, marketing materials, presentations, newsletters, brochures, advertising and email. Additionally, I waive any rights to inspect or approve the finished photographs or electronic matter that may be used and I waive my right to royalties or other compensation arising from or related to the use of this image or recording. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

By signing this release, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for purposes stated above.
