I understand that The Hairston School of Dance assumes no responisbility for injuries or illness that which I/my child mau sustain as a direct result from particpationin any activity associated with The Hairston School of Dance.
I understand and agree that by participating in any dance class, workshops, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risk and responsibility for any such injury or accident, which might occur to me or my child during any of The Hairston School of Dance classes, rehearsals, performances, or activities. I also, exempt, release, and indemnify The Hairston School of Dance, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by The Hairston School of Dance. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold The Hairston School of Dance, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of any physical limitations and agree not to exceed them. If I am agreeing to this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

By registering, I acknowledge that an inherent risk of Pandemic and Influenza exist in any public place where people are present. By attending, you on behalf of your child or children voluntarily assume all risks related to exposure to COVID-19 and other pandemic illnesses and agree not to hold The Hairston School of Dance or any faculty and staff liable.

I understand that I will keep my student home when sick or suspected of being sick. I also understand that students should not return until they are fever free for 24 hours without medication.

The Hairston School of Dance is not responsible for any personal property lost or stolen while using the facilities or while parked in the parking lot.

I give my permission to The Hairston School of Dance to use photographs, video, or audio, which may include my/ my child's image or voice for the purpose of promoting programs and serving the general public.
Should I not want my child used in any public use of photos, I will opt out of photos during regsitration.

The privacy of your personal information is very important to The Hairston School of Dance. We will not sell, rent or trade your personal information to any third party. Special care will be taken to protect the privacy and security of your information.
When you visit our Website, we do not collect any personally identifiable information without your consent. We collect and store information from you that you voluntarily choose to give us or enter on our website. When you register for classes, we collect contact information such as parent/guardian names, addresses, phone numbers, e-mail address and student information such as names, birthdays, grade, etc. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below.
