I understand that participation in any and all of Studio 517 Dance Company’s activities is not covered by the company’s medical or accidental insurance. Each participant must furnish his or her own personal coverage. I understand that dance and tumbling activities have inherent elements of risk for injury. As a participant or parent/guardian of a minor participant, I hereby save harmless and indemnify Studio 517 Dance Company and its employees from responsibility for any accident, injury or damage that may occur as a result of the participant’s acts or omissions during studio events and classes. In case of accident or sickness, I consent to emergency medical care by ambulance or hospital personnel, of whom may be contacted by a Studio 517 staff member in the event of an emergency.
