INJURY, TUITION, & PHOTO RELEASE
for Colorado Movement Lab
I hereby acknowledge and agree to the terms of this Waiver of Liability ("Agreement") in consideration for being allowed to participate in dance classes and related activities offered by Colorado Movement Lab ("CML").
Assumption of Risk:
1. I understand and acknowledge that participating in dance classes, rehearsals, performances, and other related activities involves inherent risks. These risks may include, but are not limited to, physical injury, property damage, and other potential dangers associated with dance instruction. I voluntarily assume all risks associated with my/my child's participation in these activities.
Release and Indemnity:
2. In consideration of being permitted to participate in the dance classes and activities provided by CML, I hereby release, discharge, and hold harmless CML, its owners, instructors, employees, volunteers, and related agents from any and all claims, demands, actions, or causes of action, whether at law or in equity, arising out of or in connection with my/my child’s participation in dance classes and activities, including but not limited to any injuries, damages, or losses sustained during or as a result of such participation.
I agree to indemnify and hold CML harmless from and against any and all liabilities, claims, demands, actions, or causes of action, costs, and expenses arising out of or in connection with any injury, loss, or damage suffered by me/my child or caused by me/my child to others while participating in the dance classes and activities provided by CML.
Tuition and Fees:
3. I understand that CML has established policies regarding tuition, fees, and refunds. I agree to pay all required tuition and fees. I understand that failure to comply with CML's policies may result in my/my child's dismissal from the program. Under no circumstance will Colorado Movement Lab refund or credit tuition.
Photo Release:
4. I grant CML the right to photograph, videotape, or otherwise capture my/my child's likeness or image during participation in dance classes and related activities. I understand that these photographs or videos may be used for promotional, educational, or other purposes by CML. I waive any right to inspect or approve the final products and release CML from any claims or liability arising out of the use of such photographs or videos.
Governing Law and Severability:
5. This Agreement shall be governed by and construed in accordance with the laws of Colorado. If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable.
