By checking the box below, I acknowledge that I have read, understood, and accept the terms of this Dance Studio Participation Waiver. I understand that participation in dance classes, workshops, rehearsals, and related activities at Productions Dance involves physical activity and inherent risks, including but not limited to strains, sprains, falls, and other potential injuries.

I also understand that it is my responsibility to inform the instructors of any pre-existing medical conditions, injuries, or limitations that could affect my ability to participate safely or that of the minor participant in my care.

I release Productions Dance, its instructors, staff, and affiliates from any and all liability for injuries, damages, or losses that may arise from my participation or the participation of the minor in my care in dance activities.

I agree to indemnify and hold harmless Productions Dance, its instructors, staff, and affiliates from any claims, suits, actions, or expenses related to my participation or the participation of the minor in my care.

I acknowledge that Productions Dance and its instructors are not medical professionals and any advice should not replace professional medical guidance.
