I acknowledge the risk of injury while participating in dance classes(the "Activity"), and in exchange for the right to participate in the Activity, I hereby knowingly and voluntarily waive any and all rights, claims, or causes of action of any kind whatsoever arising out of my participation, or my child's participation, in the Activity, and do hereby release and forever discharge Grace School of Dance, located at 106 E Washington Ave, Jackson, Michigan, their instructors, members, managers, agents, staff, volunteers, heirs, and assigns, for any physical or psychological injury, or property damage, including but not limited to injury, illness, paralysis, disability, death, damages, or economical loss that I may suffer as a direct result of my participation in the Activity, including travel to and from the Activity. I acknowledge that dance classes may involve trials of participants' physical and mental limits, and may include the risk of serious injury, death, and property loss. The risks include, but are not limited to, those caused by the location, terrain, facilities, weather, temperature, condition of participants, lack of hydration, equipment, vehicular traffic, and the actions of others, including, but not limited to, participants, instructors, volunteers, spectators, and event officials.

In the event that I, or my child, should require medical care or treatment for injuries or illness sustained as a result of my participation in dance classes, I agree to be financially responsible for any costs incurred as a result of such treatment. I understand that if my child acquires any injuries while participating in the Activity and I am not present, Grace School of Dance will notify me via phone call and/or text message and if I am not able to be reached, the alternate emergency contact will then be notified.

Payment is due by first of each month, no later than the 10th. A $20 late fee applies if payment is not received by then. If for any reason your child must withdraw, please inform the staff. Charges may continue if we are not notified. Costumes will be ordered after December 1; no refund available after that date. If you are unable to make payments on time, you must inform the staff so we can create a payment plan with you. I understand that my child(ren) may not be able to participate in the performances if my balance is not paid.

Photos may be taken during class or recitals and posted on either our web page or social media pages. I give my permission for my student(s) photos to be posted on Grace media.
