I acknowledge that:
• With participation in any activity there is a risk of injury, loss of property and damage.
• I and/or my child have no physical impediment to the participation applied for.
• I voluntarily assume all risks associated with my/my child’s participation in activities at or with Creator’s Arts Centre.
If I or my child(ren) have been advised by a physician to study dance for the purpose of correcting a physical problem I will advise Creator’s Arts Centre (at the onset of study) of the nature of the problem and the desired outcome. I acknowledge that I am required to provide a note from my physician to Creator’s Arts Centre stating my safe continuation in dance. Furthermore, I acknowledge that if I am pregnant, my continued participation is at me and my unborn child’s own risk.
In consideration for the Creator’s Arts Centre accepting the application for me and/or my child named above, I hereby, for myself and/or my child, as well as our heirs and personal representatives, RELEASE AND FOREVER DISCHARGE the Creator’s Arts Centre and its agents, employees and volunteers from any and all claims, actions, causes of action demands whatsoever for or resulting from any injury, loss or damage to my or my child’s personal property which may be sustained by any or all of us by coming to the Creator’s Arts Centre and participating in any activities at or with the Creator’s Arts Centre.
I understand that the intent of this form is to release The Creator’s Arts Centre from any liability and this release is valid as long as I and/or the above named child are students of Creator’s Arts Centre.
I authorize and release Creator’s Arts Centre of Kelowna BC to use any photographs and video taken of me/my child for use on their web-site, promotional or advertising publications, teaching resources and in their fundraising activities. I understand that I may withdraw my permission for future projects by writing to the Administrator at Creators Arts Centre.
If the person photographed is under 18, I certify that I am his/her parent or legal guardian and I give my consent without reservation on his/her behalf.
I authorize Creator’s Arts Centre of Kelowna BC to use my e-mail address to inform me of upcoming classes or workshops, information regarding my account, and information pertinent to my child’s participation at Creators Arts Centre. I authorize Creators Arts Centre to send me monthly newsletters and keep me informed through the use of my e-mail address.
I understand that I may withdraw my permission for future use by writing to the Administrator at Creators Arts Centre or by clicking “unsubscribe” at the bottom of the monthly newsletters.
I have read and agree with Creators' Arts Centre's tuition policy and understand that as the registering parent, I am solely responsible for all tuition payments and that being delinquent on payments may result in a suspension of service.
We have made some temporary changes at the studio to help do our part to create a safe environment for the dancers and parents. Please see below for this important information:
ONLINE CHECK IN
Every parent must fill out a COVID questionnaire each day your dancer has a class with us. This will be done through your online parent portals. A video will be sent to you before your child's first dance class so you know how to do this.
Your child’s teacher will get a notification when the form has been done. Any dancer arriving to class who does not have the form done on line will not be permitted to dance that day.
If you answered “YES” for your dancer to one of the questions included under ‘Key Symptoms of Illness’ (excluding fever), they should stay home for 24 hours from when the symptom started. If the symptom improves, they may return to dance when they feel well enough. If the symptom persists or worsens, seek a health assessment.
If you answered “YES” to two or more of the questions included under ‘Key symptoms of Illness’ or you have a fever, seek a health assessment for your dancer. A health assessment includes calling 8-1-1, or a primary care provider like a physician or nurse practitioner. If a health assessment is required, you should not return to dance until COVID-19 has been excluded and your child’s symptoms have improved.
If a COVID-19 test is recommended but is not done because you choose not to have the test, or you do not seek a health assessment when recommended, and your dancer’s symptoms are not related to a previously diagnosed health condition, your dancer should stay home from dance until 10 days after the onset of symptoms, and then they may return if they are feeling well enough.
All staff are required to do daily health check forms before starting work for the day.
As outlined on the BC government website, masks are required for everyone
entering the studio. A face shield is not a substitute for a mask as it has an opening below the mouth.
There are exemptions for:
• People with health conditions or with physical, cognitive or mental impairments who cannot wear one
• People who cannot remove a mask on their own
• Children under the age of 12
If your dancer falls under the first bullet point, please let the office know before your dancer’s first in-person class back with us so I can let the teachers know your child has an approved exception.
ENTRANCES AND EXITS
All dancers and staff will enter by the elevators and exit by the side doors. Limit of 2 people per trip in the elevator if they are from different households/bubbles. Parents are encouraged to have their child come up to the 2nd floor on their own and leave on their own but if they are unable to do that, parents must wear a mask, use the provided hand sanitizer or their own and only wait in the designated areas for pick up/drop off. Bathrooms are off limits for parents.
There will be no viewing of classes until further notice. If you have a child with a special need that requires you to be on-site in case of emergency please email the office to request a waiting spot during your child's class. All persons in the waiting rooms must remain 6ft apart from non-family members
The main waiting room next to the office will be for dancers going into class in Studio #2. The former change room will be the waiting room for dancers going into class in Studio #1. We have staggered the start times for classes for our youngest dancers to minimize the amount of bodies in the space at one time before and after classes. Please do not arrive any earlier than 5 minutes before your child's class start time and 5 minutes before the end of class to help us keep the crowding down. For dancers ages 3-8 their teacher will help them get ready to go for the day at the end of their class so when you arrive to pick them up they are all set.
CHANGING AND STORAGE
As the former change room will now be used as the waiting room space for dancers/parents waiting to pick up or go into classes in “Studio 1”. It will no longer be a place for students to change or store personal items. If possible please have your dancer in their dance clothing before coming to the studio and leave in their dance clothing. Dancers ages 8 and up will still place their shoes in the hall but take their coats and personal items into the studios. Dancers ages 3-7 will take their jackets, shoes and personal items into class with them. Dancers must limit the amount of stuff they bring to class as the storage cubbies are very small. If possible please leave all school bags and extra items in the car.
Every staff member, parent and student going up to the second floor must use the hand sanitizer provided, or their own, before using the elevator. Bathrooms will be sanitized after every use. Staff will clean for ages 3-7 and dancers ages 8+ will follow the written instructions to wipe down the bathroom after use. The whole studio will be cleaned and sanitized thoroughly each day and high-touch surfaces such as doorknobs, personal belongings cubbies and ballet barres will be cleaned between every class. There is hand sanitizer in each studio to use as often as needed.
Our studio floors have been divided into individual quadrants so each dancer has a spot that allows them to dance freely while maintaining 6ft of distance from the dancer next to them. For the time being, teachers will not be offering any hands-on corrections unless absolutely necessary (for example, a dancer is falling out of a cartwheel and may hurt themselves).
Creator's Arts Centre will follow all guidelines provided by WorkSafe BC in order to keep our environment as safe and clean as possible for our dancers and instructors. That being said we simply cannot reduce the risk to zero.
We will hold all classes in person when it is safe to do so and online if we are required to. No refunds or discounts will be offered for online classes.
