If I or my child(ren) have been advised by a physician to study dance for the purpose of correcting a physical problem I will advise Creator’s Arts Centre (at the onset of study) of the nature of the problem and the desired outcome. I acknowledge that I am required to provide a note from my physician to Creator’s Arts Centre stating my safe continuation in dance. Furthermore, I acknowledge that if I am pregnant, my continued participation is at me and my unborn child’s own risk.

In consideration for the Creator’s Arts Centre accepting the application for me and/or my child named above, I hereby, for myself and/or my child, as well as our heirs and personal representatives, RELEASE AND FOREVER DISCHARGE the Creator’s Arts Centre and its agents, employees and volunteers from any and all claims, actions, causes of action demands whatsoever for or resulting from any injury, loss or damage to my or my child’s personal property which may be sustained by any or all of us by coming to the Creator’s Arts Centre and participating in any activities at or with the Creator’s Arts Centre.

I understand that the intent of this form is to release The Creator's Arts Centre from any liability and this release is valid as long as I and/or the above named child are students of Creator's Arts Centre.

I authorize and release Creator's Arts Centre of Kelowna BC to use any photographs and video taken of me/my child for use on their web-site, promotional or advertising publications, teaching resources and in their fundraising activities. I understand that I may withdraw my permission for future projects by writing to the Administrator at Creators Arts Centre.

If the person photographed is under 18, I certify that I am his/her parent or legal guardian and I give my consent without reservation on his/her behalf.

I authorize Creator's Arts Centre of Kelowna BC to use my e-mail address to inform me of upcoming classes or workshops, information regarding my account, and information pertinent to my child's participation at Creators Arts Centre. I authorize Creators Arts Centre to send me monthly newsletters and keep me informed through the use of my e-mail address.

I understand that I may withdraw my permission for future use by writing to the Administrator at Creators Arts Centre or by clicking unsubscribeat the bottom of the monthly newsletters.

I have read and agree with Creators' Arts Centre's tuition policy and understand that as the registering parent, I am solely responsible for all tuition payments and that being delinquent on payments may result in a suspension of service.

At Creator’s Arts Centre, we love to capture and share the joy and achievements of our talented young dancers. To ensure that we can continue to celebrate their accomplishments while respecting your privacy and the safety of your children, we kindly request your consent to use images and videos of your child on our social media platforms and promotional materials.

Please read the following terms carefully.

I, grant permission to Creator’s Arts Centre to use images and videos of my child, for the following purposes:

1. Posting on the official Creator’s Arts Centre website.
2. Sharing on Creator’s Arts Centre’s social media accounts (including but not limited to Facebook, Instagram etc) for promotional and educational purposes.
3. Inclusion in printed promotional materials, such as brochures, flyers and posters.

I understand that these images and videos may be shared publicly and may be accessible to a wide audience.

I also acknowledge that Creator’s Arts Centre will take reasonable measures to ensure the safety and privacy of my child and will not use any identifying information; such as full names or addresses, with the images and videos.

I understand that I may revoke this consent at any time by providing written notice to Creator’s Arts Centre via email or in person.

Thank you for your cooperation and support in allowing us to showcase the talents and achievements of our young dancers. Your child’s safety and privacy are important to us, and we will continue to prioritize them in all our activities.
