By submitting this digital signature, I agree that I understand all financial obligations for the upcoming summer dance session. I agree to be held financially responsible for payment of this account and to make payments to this account per the policies of Amazin Grace Dance Studio. I understand that all payments are due at the time of registration and are non-refundable and non-negotiable. I understand that failure to make payments to this account will result in either temporary or permanent suspension of lessons for the dancer(s) associated with this account until all payments have been made in full. Additionally, I agree that if my account becomes past due with summer session payments, my child will not be permitted to register for fall classes.

I agree to comply with the Amazin’ Grace Dance Studio general studio policies stated online. I agree to comply with studio policies listed in the Covid-19 Policies and General Policies online. My child and I understand that participation in dance involves potential risks. I, (the parent/guardian), assume all risks associated with participation in dance classes, performances, rehearsals and events. These risks include, but are not limited to falls, exposure to communicable diseases such as Covid-19, contact with other persons or any other reasonable risks associated with dance. All risks to my child/self are known and understood by me, and therefore I agree to not hold Amazin’ Grace Dance Studio liable if I/my child were affected by such risks.
