I hereby grant permission for myself and/or my student to be photographed, videotaped, and/or interviewed during the course of the year by Wayne Ballet & Center for Dance Arts and any of its authorized agents, and consent to the publication, broadcast or other use for the purpose of promoting WBCDA.

Wayne Ballet & Center for Dance Arts and its instructors are not liable for personal injuries, nor loss or damage to personal property. In addition, intending to be legally bound for myself, my herd, executors and administrators, I release WBCDA and any parties acting on their behalf and with their approval, from liability if I or my student should become injured. Wayne Ballet will not and may not dispense any medications and/or medical treatment to any student. Wayne Ballet and its employees may not be held liable for any sickness that can occur as a result of airborne illnesses.

Tuition, Camp Fees, and Registration Fees are Non-Refundable. Once registration is complete and tuition paid for, we not only hold a place for that student, we also commit to hiring a dedicated teacher for that session. If a scheduling conflict should occur, we are able to issue a credit on an account to be used towards a future session/camp (must be used within one year). Tuition is due a week prior to the start of the session and classes can be denied to students who have past due tuition.
