A breakdown:
1. Tuition is due on the 1st of the month, auto pay charged on the 6th, late fees charged on the 10th. Tuition is based on a full month, or a full 9 month season, depending on your class type.
2. Our front office is mainly staffed with volunteers. If you need help, have account questions, or want a tour, please communicate with our head staff via BAND, email, or text.
3. Withdrawals from our preschool classroom or our company after Oct 15th are charged a withdrawal fee.
4. Auto pay is required to have set up, but you may pay with cash, check, or venmo before the 6th of the month.
5. We have a strict code of conduct that our students, staff, and parents must uphold.
6. In class observation is not allowed in our studios/classrooms unless it's a parent involved class or in special circumstances. Please see our observation guidelines in our handbook.
7. We use a communication system called "BAND" where all of our announcements are made. YOU NEED THIS to know what's happening.
8. Classes may have required items/dress codes/supplies, please make sure to read class descriptions to know what you're supposed to have/bring.
9. If you don't receive an email with your 1st day information, your class is not full enough to run yet. Please check spam folders for your 1st day welcome email.
10. Company members are required to follow a very specific handbook, keep good attendance, be a role model, take extra classes, travel, etc. PLEASE make sure to read through your specific handbook to know what's required of you.

I understand this form enrolls this student in a program that could involve: strenuous physical activity, exposure to art supplies, &/or snacks. I hereby affirm that the student is in good physical condition. I have listed any disability or illness. In consideration of myself, my heirs & assigns, I hereby release Jackie Frazier, her family or employees, & all independent contractors from any claims, deems, & causes of action arising from my or the above named person’s participation in any R & R programs. I hereby release Jackie Frazier, her family or employees, & all independent contractors, from any liability now, or in the future for any injury however caused occurring before, during, or after participation in any R & R programs offered at any time, while in the vicinity of the premises of the above stated business, or in any activity sponsored, represented, or organized by Jackie Frazier, her family or employees, & all independent contractors for any reason. This form authorizes an R& R employee to secure emergency medical care for the student listed above if emergency contact cannot be made (guardian is responsible for emergency medical charges). This form also authorizes basic first aid/CPR care to be delivered by a certified individual. By signing, I hereby affirm that I have read & fully understand & agree with the above waiver.

I understand that the registration fee must be paid to hold my spot in class. I understand that tuition is due on the first of the month. I understand the withdrawal policy and acknowledge that my account will be charged all appropriate class fees (including late fees). I agree to follow the above parent policies and have my child follow the code of conduct.
