Release Forms

If you have not filled out the new release forms for the current semester, please go to the 'Waiver and Release Tab' and sign our "Activity Release" & "Photo Release"/"Photo Opt-Out" forms. These forms are only good for a semester at a time, so please remember to get those signed as soon as possible.
